Athletic Participation/Parental Consent/Physical Examination Form

This REQUIRED FORM is available in the Student Activities Office or on our website in Sports. Submit the completed form to the Activities Office before the first day of try-outs. No one may participate without clearance of his/her physical exam (done after May 1st for the upcoming year) by the Athletic Trainer. Make sure your Tetanus shot is up-to-date (within the last 10 years). Please write which sport you are trying out for at the top of the front page. Keep a copy at home.

Student Accident Insurance

No student will participate in any athletic activity without evidence of insurance coverage. Inexpensive Student Accident Insurance is available for purchase through the school for individuals who do not have insurance or wish to buy additional insurance. Forms are available in the Student Activities Office.

Team Managers

Team Managers assist the head coach for some sports with equipment handling, record keeping, and other duties. Any student may apply to the head coach of a sport for the position of Team Manager.

Athletic Awards

An Athletic Awards ceremony is held three times per year at the end of each season. Athletes are awarded Varsity Letters or Certificates of Participation, as determined by using criteria set by the coach. Athletes who earn a GPA of 3.0 or higher for the past quarter AND letter in a varsity sport will receive a VHSL Honor Roll Certificate. In addition, those athletes whose GPA is 3.5 or above also receive a Northern Region Honor Roll Certificate.

Game tickets are sold at the gate at a cost of $4.00 for adults and $2.00 for students with a Student ID for the following games: Freshmen and Junior Varsity Football (these games are played as double-headers and require only one ticket), Basketball, Baseball, Field Hockey, Soccer, and Lacrosse games.
