Families or guardian(s) are requested to return the following to their school for completion of the application process:
- Homebound Instruction Application Form| Solicitud Para Educación Domiciliari—Español
- Medical Certification of Need
- APS Release of Information
Please return the completed forms to:
- Elementary: return to the school principal
- Secondary: return to the school principal or director of counseling services
Where to submit:
School teams are required to submit these forms with the School Information Form.
Following an extension for Homebound services, a return to learn plan must be filled out and submitted:
