Homebound Instruction Forms

Families or guardian(s) are requested to return the following to their school for completion of the application process:

1. Homebound Instruction Application Form|  Solicitud Para Educación Domiciliari—Español
2. Medical Certification of Need
3. APS Release of Information

Please return the completed forms to:

• Elementary: return to the school principal
• Secondary: return to the school principal or director of counseling services

School teams are required to submit these forms with the School Information Form on the Staff Homebound Google Site.

Following an extension for Homebound services, a return to learn plan must be filled out and submitted:
