Intervention Plan – Name and ID Number
This section states the name of the student, name and area of the plan, and the start and end date for plan. It also states the person who wrote the plan “created by” and the “intervention provider.” Please note: the plan creator may or may not be the staff member who is delivering the intervention to your child.
Reason for Intervention and Parent Contact
This section states the general reason your child is receiving an intervention and in which subject (Reading, Math, SEL/Behavior, Attendance). You will also see the date you were contacted. This section includes “notes” which will have additional information or strategies, resources and materials to support your student’s growth at home.
Area of Focus/Goals
This section tells you the subject of focus in either the area of “math”, “reading”, “SEL/Behavior”, or “attendance.” It includes the specific area of need in the academic area, and the specific goal(s) and objectives associated with the area of need. To help families understand the amount of support their student will receive, this section includes the length of each session (minutes), how many days a week it meets, and the start date of the intervention. Next you will see the term “tool”. This refers to what the teacher will use to monitor your child’s progress and how often it is used.
Progress Monitoring
This section explains how much progress your child has made towards their goal and the targeted end date of the intervention. “Goal Status” will tell you if the student is still working toward their goal, has met their goal, or has not met their goal. The visual in this section represents the data collected during progress monitoring. The blue line is your student’s progress and the beige line is expected growth over time. Data points on or above this line means your student is on target to meet their goal(s).
Intervention Attendance
This section tracks your child’s participation in the intervention. This includes the total number of absences (when the student did not attend the intervention, though they may have been present in school) and the total number of hours and minutes that they have participated as of the date of sharing the report. This section also includes the name of the intervention and attendance related notes.
Outcome and Next Steps
This section reviews whether the goal has been met, and the next steps (“action”) the teacher will take based on your student’s performance.
Student Plan History
If this section is included, it lists the name and area of past interventions your student received with the goal and goal status.
